2008 Melbourne International Artist Card Application Form

If you are a professional practising artist, or someone working in the artistic community, apply to receive a 2008 Melbourne International Arts Festival Artist Card by completing the Application Form below.

If you do not wish to apply for the card online, please call us on 03 9662 4242, write to us at artistcard@melbournefestival.com.au, or send a stamped, self-addressed envelope to Artist Card, PO Box 10 Flinders Lane, Melbourne VIC 8009 with a letter requesting an application form.

Your Artist Card will be posted to you so please provide us with a complete current postal address. If, for any reason, you do not wish your card to be posted, please mark the address fields on the form below not applicable and email artistcard@melbournefestival.com.au to arrange collection from the Festival office.
